What is testosterone pellet therapy?

Testosterone pellet therapy is a hormone replacement method in which small pellets containing testosterone are surgically implanted under the skin to release the hormone gradually over three to six months.

Testosterone pellet therapy delivers hormone replacement through small, solid implants placed just beneath the skin. A provider makes a small incision (typically in the hip or buttock area) and inserts one or more pellets subcutaneously, then closes the site with a stitch or surgical tape. Each pellet is roughly the size of a grain of rice and contains crystalline testosterone.

The pellets dissolve slowly over three to six months, releasing testosterone steadily into the bloodstream. This continuous delivery differs from injections, which create peaks and valleys in hormone levels, or topical treatments that require daily application. Patients do not need to remember daily doses or schedule frequent clinic visits for injections during the treatment window.

Pellet therapy is often chosen by men whose symptoms of low testosterone persist despite other treatment methods, or who prefer the convenience of a single procedure spread across several months. The implant site may feel tender for a few days after insertion, and some patients experience minor bruising.

Finding experienced providers matters, as insertion technique and pellet selection affect treatment outcomes. If you are exploring this option, review TRT providers in your area to compare clinics offering pellet implantation services.
