What is hematocrit?
Hematocrit is a blood test that measures the percentage of red blood cells relative to total blood volume, expressed as a percentage.
A hematocrit test shows what percentage of your blood consists of red blood cells, with normal ranges typically between 38.8 and 50% for men. The measurement is expressed as a percentage or hematocrit count (Hct) and appears on standard lab work ordered during testosterone replacement therapy monitoring.
Testosterone directly stimulates the bone marrow to produce more red blood cells, a process called erythropoiesis. This means hematocrit naturally rises during TRT, particularly in the first few months of treatment. Higher red blood cell volume increases oxygen delivery to tissues but also thickens blood, raising the risk of clots, stroke, and cardiovascular events if levels become too elevated.
TRT providers track hematocrit at baseline and throughout treatment, typically checking it at 3 months, 6 months, and annually after initial titration. Most clinics watch for hematocrit above 54%, though thresholds vary based on individual health history. If levels climb too high, adjustments to testosterone dose or dosing frequency often bring readings back into safe ranges without stopping therapy entirely.
